Roles of Motorcycle Type and Protective Clothing in Motorcycle Crash Injuries

Abstract: Background. The aims of this study were to identify subgroups of motorcyclists with a higher accident risk and evaluate the efficiency of protective clothing for preventing injuries. Methods. A 1-year prospective study of motorcycle crashes was conducted beginning in June 2012. Participants were patients involved in motorcycle crashes and admitted to our emergency department. Results. A total of 226 patients were included in the study. In total, 174 patients were involved in crashes with light motorcycles. Patients involved in a motorcycle accident without a helmet had a higher incidence of head and maxillofacial trauma. Motorcycle jackets were not protective for systemic injuries ( ) or upper extremity fractures ( ). Motorcycle pants ( ) and motorcycle shoes ( ) were not protective against leg and foot fractures. However, motorcycle protective clothes were protective against soft-tissue injuries ( ). Conclusion. Riders of heavy motorcycles rode more safely than riders of light motorcycles. Light motorcycle riders were the most vulnerable and comprised the largest percentage of motorcyclists. Helmets may be effective for preventing head and facial injuries. Other protective clothes were not effective against fractures or systemic injuries. 1. Introduction Motorcycles are the fastest growing sector of motor vehicles worldwide and comprise the majority of all motor vehicles in low- and middle-income countries [1]. Motorcycles are an important part of social life in high-income countries [1]. Both heavy and light motorcycles are used in Turkey. A recent study revealed that heavy motorcycles tend to be used in summer and light motorcycles are used mostly during spring and fall. Light motorcycles are generally used for work and transport, whereas heavy motorcycles are preferred for vacation and travel [2]. Different rider characteristics are related to the various types of motorcycles and motorcycle-related accidents. Motorcyclists are a vulnerable group of riders and have a particularly high accident risk [2]. Thus, there is a need for research to identify risks for the motorcyclist during a crash. The aims of this study were to identify subgroups of motorcyclists with a higher accident risk and evaluate the efficiency of protective clothing for preventing injuries [1]. 2. Methods A 1-year prospective study of motorcycle crashes was conducted beginning in June 2012. The participants in the study were patients involved in motorcycle crashes and admitted to our emergency department (ED). Participants were interviewed in the ED after their initial management.

Abstract:
INTRODUCTION: SAM (systolic anterior motion) is defined as the dynamic motion of the anterior mitral valve leaflet to the left ventricular outflow tract (LVOT) during systole. In this study, we present a case which we encountered with significant left ventricular outflow tract obstruction (LVOTO) related to SAM and developed after Ross-Konno operation. METHOD: A 4-month, 6.2 kg girl underwent balloon valvuloplasty due to bicuspid aortic stenosis when she was one-month. Ross-Konno operation was performed on the patient due to residual aortic stenosis (peak gradient of 140 mm Hg) and severe aortic regurgitation. Intraoperative echocardiography (ECHO) showed a peak gradient of 25 mmHg between the LV-Aorta. RESULTS: In the ICU at postoperative 3. hour, when tachycardia began with the patient’s hemodynamic deterioration, SAM in ECHO and concomitant LVOTO (Peak grad: 140 mmHg) and 2nd degree MY was identified. Pulmonary edema was observed along with a decrease in oxygen saturation. The treatment initiated for removing the patient from hyperdynamic state and reducing the volume deficit. Sedation and analgesia were increased. The low-dose dobutamine was stopped. Tachycardia was attempted to take control with beta-blockers. The same treatment was continued on day 2, but the frequency of patient’s tachycardia has not been reduced despite high doses of beta blockers. Heart rate was controlled by Amiodarone. On day 4, mitral regurgitation was slightly decreased while the LV-Aorta gradient was measured between 50-60 mmHg. On day 5, SAM has diminished greatly (20 mmHg). The patient was extubated 6th day and was discharged on the 12th day without any problem. One year after, LVOT gradient was not detected in the ECHO. DISCUSSION: In adult patients, although a variety of surgical procedures for the removal of SAM had been reported, no experience in infants exists in the literature on these techniques. Despite intensive medical treatment, it may take longer than expected. We think that also the decline of existing preoperative LV hypertrophy in this period contributes.

Abstract: This paper reports on a guided navigation system built on the textbook search engine developed at Anadolu University to support distance education students. The search engine uses Turkish Language specific language processing modules to enable searches over course material presented in Open Education Faculty textbooks. We implemented a guided navigation engine by using query log mining to our application. It makes use of previous users’ sessions to help students find information they are looking for by using fewer queries. We used item-based similarity to do query recommendation. This paper describes the search application, query expansion and evaluation of the system over existing query logs. We show that our system suggested relevant queries with a success rate of 85%.
Sustainability in higher education: A needs assessment on a course “Education and awareness for sustainability”

Abstract: The purpose of the study was to make an assessment on the needs of university students regarded to be aware of environmental problems and sustainable development concept introduced in an undergraduate course. As a first part of course evaluation project, the needs assessment (NA) study comprising three steps was realized with 85 undergraduate students enrolling in the course Education and Awareness for Sustainability. NA questionnaire including both open-ended and closed-ended items was used for data collection. The responses to open-ended questions were subjected to content analysis whereas those to closed-ended items were subjected to statistical analysis. As the results suggested, students believed that education was the fundamental stage to constitute awareness for sustainability. The students indicated that the use of methods of discussion and brain storming were the best methods, and the instructional media such as computer and projector, video, and OHP were essentially important for developing awareness for sustainability. Almost all the students reported that the course aiming to introduce education and awareness for sustainability was beneficial for them to integrate the concept to their own profession and to develop awareness and sense of responsibility for protecting environmental resources.
THE EFFECTS OF AEROBIC EXERCISE ON SKELETAL MUSCLE METABOLISM, MORPHOLOGY AND IN SITU ENDURANCE IN DIABETIC RATS

Abstract: The effects of aerobic exercise training on skeletal muscle endurance capacity were examined in diabetic rats in situ. Moderate diabetes was induced by iv injection of streptozotocin and an exercise training program on a treadmill was carried out for 8 weeks. The animals randomly assigned to one of the four experimental groups: control-sedentary (CS), control-exercise (CE), diabetic-sedentary (DS) or diabetic-exercise (DE). The changes in the muscle endurance capacity were evaluated through the square wave impulses (supramaximal) of 0.2-ms duration at 1 Hz in the in situ gastrocnemius-soleus muscle complex. Muscle was stimulated continuously until tension development reduced to the half of this maximal value. Time interval between the beginning and the end of stimulation period is defined as contraction duration. Following the training period, blood glucose level reduced significantly in the DE group compared to DS group (p < 0.05). The soles muscle citrate synthase activity was increased significantly in both of the trained groups compared to sedentary animals (p < 0.05). Fatigued muscle lactate values were not significantly different from each other. Ultrastractural abnormality of the skeletal muscle in DS group disappeared with training. Presence of increased lipid droplets, mitochondria clusters and glycogen accumulation was observed in the skeletal muscle of DE group. The contraction duration was longer in the DE group than others (p < 0.001). Fatigue resistance of exercised diabetic animals may be explained by increased intramyocellular lipid droplets, high blood glucose level and muscle citrate synthase activity
Hallucination: A rare complication of levetiracetam theraphy

Abstract: Levetiracetam is a new antiepileptic drug. In addition to epilepsy, it is also used for treating anxiety disorders and dystonia as well as tardive dyskinesia associated with the use of levodopa and neuroleptic drugs. Phenytoin therapy in a 10-year-old boy with convulsions was discontinued following cardiac rhythm impairment. The patient was then started on levetiracetam. However, visual and auditory hallucinations were observed on the 1st day of levetiracetam therapy. Levetiracetam was discontinued and replaced with sodium valproate, and the hallucinations resolved. The purpose of this report was to remind physicians that hallucinations are one of the rare complications of levetiracetam
Using terlipressin in a pediatric patient with septic shock resistant to catecholamines

Abstract: Sepsis and septic shock are important causes of morbidity and mortality in critically ill children. The goal of treatment is to ensure adequate mean arterial pressure to maintain organ perfusion. The growing number of instances of peripheral vascular hyporeactivity to catecholamines has necessitated the search for alternative vasopressors. A 14-year-old boy had septic shock, with a high cardiac index and low systemic vascular resistance index (SVRI) measurements according to pulse contour analysis, despite treatment with dopamine, dobutamine, adrenaline, and noradrenaline infusions. A terlipressin (TP) 10 μg/kg intravenous bolus was administered, followed by a 1 μg/kg/minute continuous infusion. The response to TP treatment was assessed using pulse contour analysis. The mean arterial pressure and SVRI increased, and the cardiac index and heart rate decreased within 10 minutes after bolus administration of TP. Noradrenaline infusion could be reduced to 0.7 μg/kg/minute within 5 hours. Abstract: It is shown that exciton condensates exhibit an incredible richness in the role that fundamental symmetries play whether manifest or broken. We investigate the appearance of the singlet and the triplet excitonic condensates under spin-orbit coupling (SOC) and consider realistic couplings to the radiation field as manipulated by the fundamental symmetries related to spin, time, orbital and particle degrees of freedom. The role of the strong SOC in inducing a ground state with coexisting singlet and triplet components is shown. It is also shown that the ground state of an excitonic condensate, in the realistic senario of spontaneous coupling to the radiation field, is dominated by a dark component transparent to photoluminescence.

Abstract: We consider non-selfadjoint operators of the kind arising in linearized NLS and prove dispersive bounds for the time-evolution without assuming that the edges of the essential spectrum are regular. Our approach does not depend on any specific properties of NLS. Rather, it is axiomatic on the linear level, and our results are obtained from four assumptions (which are of course motivated by NLS). This work is in three dimensions.
Max-Margin Stacking and Sparse Regularization for Linear Classifier Combination and Selection

Abstract: The main principle of stacked generalization (or Stacking) is using a second-level generalizer to combine the outputs of base classifiers in an ensemble. In this paper, we investigate different combination types under the stacking framework; namely weighted sum (WS), class-dependent weighted sum (CWS) and linear stacked generalization (LSG). For learning the weights, we propose using regularized empirical risk minimization with the hinge loss. In addition, we propose using group sparsity for regularization to facilitate classifier selection. We performed experiments using two different ensemble setups with differing diversities on 8 real-world datasets. Results show the power of regularized learning with the hinge loss function. Using sparse regularization, we are able to reduce the number of selected classifiers of the diverse ensemble without sacrificing accuracy. With the non-diverse ensembles, we even gain accuracy on average by using sparse regularization.
